Insurance & Fees

Using Insurance

Coverage and cost depends upon each patient's individual health insurance plan.  We do not guarantee coverage or payment by insurance. Please contact your plan to determine your out of pocket costs

Accepted In-Network Insurance Plans

We will bill In-Network insurances on your behalf, and pass any costs determined by insurance on to you

  • Regence

  • Premera Blue Cross/Blue Shield

  • Kaiser PPO

  • Kaiser HMO (Dr. Petrey only)

  • Aetna

  • Lifewise

  • FirstChoice

  • Ambetter

Out-of Network Insurance Plans

Payment is due at time of service

  • United Healthcare

Medicare

  • Medicare will allow chiropractic spinal adjustments for acute injuries.

  • Medicare does not cover the intake exam, Active Release Technique / soft tissue treatment, or chiropractic adjustments to the arms and legs. 

  • You will be provided an ABN and self-pay rate for uncovered services.

Motor Vehicle Accidents

PIP coverage only

  • We accept open Personal Injury Protection claims (through your OWN insurance, regardless of fault)

  • We do not accept third-party claims (though the other driver's insurance)

  • Cases with open liens or exhausted PIP funds will have payment due at the time of service and all necessary documents will be supplied to support your settlement.  

Self-Pay and Fees

Paying for treatment out of pocket

A Time of Service rate is available to anyone paying at the time of service.  A Good Faith Estimate for each type of service is available.

Admin Fees

Administrative fees are charged on an ad hoc basis for requests that occur outside appointment times:

  • Emails requesting clinical advice 

  • Phone Consults in lieu of appointments

  • Research / document review / second opinion etc.

Cancellation Fees

24-hr cancellation policy

Due to the one-on-one nature of our appointments, appointments may be cancelled/rescheduled online up to 24 hours before the start time.  We ask that you notify us by phone or email for any last-minute cancellations so that we may offer the appointment to another patient.

  • Cancellations with less than 24-hr notice will be charged a late cancellation fee

  • Missed appointments without notice will be charged the full time-of-service fee
